Application Instructions

 

To request admission to the English Language Institute: 

1. Complete and submit this form to the ELI by clicking on the submit button at the bottom of the form. (Do not email this page to us; use the submit button. Emailing will only result in our receiving a blank form.)

and

2. Send the following items via mail:

  • A non-refundable $100.00 application fee. We accept bank checks or money orders by mail. Visa, Mastercard, or American Express are also accepted for on-line payments.
  • Formal proof that the applicant has completed secondary education (transcript or copy of diploma)
  • Proof of financial responsibility if applying for a student (F-1) visa (Certificate of Financial Responsibility and certified bank statement from applicant's or guarantor's bank, or statement from sponsoring agency--in English, showing U.S. currency equivalence)
  • A copy of the applicant's passport
  • Immunizations:
    U.S. immigration laws require visiting students to the United States to submit a record of their immunizations. International students at ELI must prove they have had the following:
    • A current PPD skin test, taken within the last year, that confirms they are free of Tuberculosis
    • AND

    • One of the following vaccine combinations is acceptable
      • 2 MMRs
      • 1 MMR + 1 Measles
      • 2 Measles + 1 Rubella
      • A positive Titer blood test with lab report
    Immunization Form (Please send the completed Immunization Form only to the ELI )
    Please read the Immunization instructions before filling out the form
  • If the student is not yet 18, a parent or guardian MUST sign and date the Waiver of Liability so that the student may participate in cultural activities AND the Immunization Form, Section A, below #5, AND Section F at the bottom of the page.
  • All students must provide proof of health insurance. If you purchase health insurance in your country, the company MUST fill out the Insurance Verification Form. All responses to this form must be "yes." If you wish to be sure your insurance is acceptable, it is preferable to purchase it when you arrive at the ELI.
