This is to certify that the Guarantor named below will be fully
responsible for all expenses of the student named below during his/her studies at the
University of Florida's English Language Institute. The following should be filled out by
the Guarantor. Submitting this information constitutes acceptance of this agreement.
Students intending to study on a student (F-1) visa should fill this information and
submit a completed Certificate of Financial Responsibility (CFR). Non-F1 students do not
need to submit a CFR, but should fill out the information below.

and of my medical records to any qualified personnel for treatment in case of illness of
